We see the very same issues in our area. Basically, like the fire service, most EMS services are not honest with their taxpayers as to the true costs of providing the proper standard of care in 2014. This is not a new issue, this is the norm. In Maine the powers that be, in their infinite wisdom adopted (bought) an EMD plan and require the dispatch centers to air the appropriate determinant codes, but failed to train the EMS agencies on how to implement them or how to address what is expected when a Delta level call is received and the service does't have ALS available. Mot days at least 75% of the VAC's ignore the EMD codes, go to the see with BLS then determine if the EMD code was appropriate and request ALS from their preferred mutual aid partner (the one's that won't charge).

In the end the cost of providing the Standard of Care in 2014 is being pushed down to the responder levels, where the EMT on scene must make a determination on the level of care the patient receives and money is a factor. Those people in administrative levels of EMS services ignore their responsibility to properly insulate their responders from needing to factor in costs when providing care. When you have a patient in your bus, you should only be making decisions based on the quality of care that's most appropriate for the patient.

EMDing is good. You just need the staff to do it. When you can't EMD here I know they send it all. Aka ambulance and medic. Is that a national standard ? I do know of the county protocol about being an ALS agency and not being able to EMD a call goes ALS (sorry I know it's vague but it's a long protocol). And also not talking to the caller is also ALS and some other weird situations that happen daily.

More than a third of these agencies use paid personnel to insure they have a crew.

You can add both Cortlandt and Mohegan to your list in terms of contracted staffing.

So it is closer to 50% of the agencies in the county are paying people for coverage.

If they did an analysis and staffed according to volume and response times instead of antiquated, arbitrary jurisdictional boundaries, there would probably be a great improvement in the level of service provided with no increase in costs.

If an agency pays for one EMT and can't get the second crewmember, they're really just pissing away money.

Too bad there is no oversight in EMS. NYS BEMS is a toothless tiger and the regional EMS councils don't do anything.

I agree with everything, except your last line.

NYS BEMS has teeth, they just don't use them. They have the legal authority to revoke a CON for failing to cover the calls. I think the state is concerned that if they start enforcing this, they may eliminate a vast majority of services.

The Westchester EMS council has tried very hard to address the issue, but is very restricted by state regulations as to what it can do. It also gets minimal support from the EMS community. What would you like it to do that its not doing, (that it legally can)?
