  2. ALS - Wake County North Carolina is a perfect example of one of the best third services in the country. In fact, a starting paramedic makes close to 10 grand more than a city of raleigh probie with county benefits (no 20 and out though). My understanding is that the FD EMS relationship is absolutely amazing - EMS is treated as equal professionals and FD is consistently looking to do/learn more in terms of patient care. So, the whole benefits and compensation package is possible - especially here. But you and i will never agree about that :)

    Besides, if we were to sell off some of the useless/replicated fire apparatus throughout the county we should have more than enough start up capital for Westchester County EMS. :P

    Goose, I have to agree with you on this. I was lucky enough to go down and ride with Wake County EMS for a long weekend while in Paramedic school. Their relationship with FD is amazing and while their system could totally work here there is alot that needs to change in our goverment to get rid of the jurisdictional beefs between organizations. The idea of a county run EMS system, full compensated ambulance, with the addition of "voluntaries"(also fully compensated) is one that doesnt fall far from a beginning in Westchester. One of the Med. Directors for Wake County is originally from Westchester and worked the EMS system here for quite a while. It took NC changing its laws to make the counties responsible for EMS coverage and cities and towns responsible for FD coverage. This same kind of legislation could be what turns NY around as far as EMS is concerned.

Actually your area does have EMD protocols however they are severely under used, 911 callers get appropriate pre-arrival instructions but nothing has been put in place in regard to assigning calls priorities and basing resource dispatched based on that. With that said I'm not sure that I would be a fan of such a protocol for the area being discussed now. Probably would be a good idea to begin with SOP/SOG's on a local level that handles response modes then work with control in regard to resources dispatched based on priority assigned.

    I don't quite remember that protocol or it every happening once while I was on duty but it still makes no sense to me at all. It may also couple with the fact that the majority of us who work there rarely utilize medevac services.

    The medic on that job is a very experienced firefighter/paramedic with years of service in both fields. There also was another FF/Paramedic who was coming off duty that assisted, resulting in good decision making and excellent, quality care.

    Tom, what I meant by saying we dont have protocols is that we dont get different responses based on the EMD priority coding, i.e, a call coded ALS gets the same recommendation as a BLS coding. The EMD is getting done when we get the calls direct at control. So the foundation is layed to use the EMD system, but like you said the SOP/SOG's either arent in place or dont exist.


  6. What was meant by auto launch was that the medic was not on scene yet but made request for statflight based on the fire chief's(2341) report, he was on scene and witnessed the car vs ped. Based on the report given from the chief to paramedic responding the medic(39M1) requested air1 be auto launched. In retrospect it was great call by the medic, the girl stopped breathing on the flight down and was tubed when they made the ER. I heard rumors she underwent brain surgery yesterday at WCMC. Our area doesnt have EMD protocols which designate types of response as of yet.


  7. 585...there was a city water dept rep on scene for some of the incident so it is a situation that the city has been and is aware of. Water is a definite issue in certain parts of the city, ie....strip mall fire and hampton oaks back in the days.

    Monty...Mohegan FAST put to work as far as exterior operations with hoselines. With the couple of structural collapses an interior attack was cut off. Mohegan also put a good amount of guys in the street which helped us out too.

    3 story multi dwelling fully involved on all floors, Aand D side large volume of flame,on arrival. extreme water problems due to water main size, all hydrants in and around the scene with low pressure and volume. Structural collapse of top floor of A side and part of B side. Once good water was established fire was K/D in good time. Mohegan FAST put to work. As well as Mohegan and Montrose engines aid in relaying substantial water to units on scene. FFs on scene got their butts kicked during initil attack due to amount of fire and lack of good water on arrival. Unknown cause so far. Good job by mutli agencies to get together to put this one out.

    There is no way that Fairview should have been dispatched to that call.  Elmsford covers between exit 1 and exit 4.  Mutual aid....ok....primary....no way

    If you listened to the initial dispatch the original location of the incident was well inside Fairview's coverage area. I287 EB mile marker 3.0(ramp from RT119-tarrytown rd) is covered by Fairview FD(per their own protocols) when the call was update by thruway state police to I287 EB at exit 3 it then was put into Elmsford FD district, so yes 60 control is for real and it is easy to sit in a room with a scanner and critique that which you do not know but thanks for your concern
