1. The response policy is gonna have to be setup by the EMS executives and discussed with the fire cheifs, contractor, OVAC tri village ALS, CPD and 60 Control.

    1. Have paid emt/driver wait in HQ and tone out.

    2. Have paid member respond to scene and tone out. If not covered send M/A or ask ALS to ride it in (if its even an ALS call per EMD)

This is not going to work. I've known other agencies who tried to go cheap & wound up eating the contract amt. You don't get reimbursement (ins. $) unless Y0U transport. The other agencies also thought that they could crew at least one other person. Also how is the paid staffer going to get to the scene, fly car or ambulance. Going alone with the ambulance & then try to explain that you can't transport is a PR disaster waiting to happen. The other agencies I referred to (after paying out the contract amt & not recouping the $) hired another staffer & while they didn't make a lot of extra $ they did cover staff expenses. Someone is not thinking in Croton.

  2. seth from my understanding its the village that wants the paid EMT from a commerical agency. so there is no conflict of intrest

Can't work & volunteer for the same agency. Also hiring one person is a waste of money. I know of a few agencies who tried this way in order to save some money & wound up eating that yrs. salary. You only get paid back if you transport. The agencies couldn't get the second person in order to get the rig out.

  3. From time call is received to tones going out = processing time. From tones to the time needed resources begin to respond (Ambulance, Engine etc. leave quarters not just chiefs car.) = Assembly time. From time resources leave quarters to time on scene = travel time. Processing time + Assembly time + travel time = Total response time. We have control, or at least, are able to know all of these different pieces. It would take time (no pun intended) to colate that info so we could see where (if any) area can be improved.

  4. Our BLS 1st Responders are trained and equipped with Epi pens to help patients in anaphylaxis and albuterol to treat asthma. Anyone suffering from anaphylaxis or asthma generally has little or no control over these medical conditions.

While this may sound a little cold, but at what point do we stop taking responsability for the stupidity of others? You play russian roulet with heroin and you may take a bullet. Why is it our responsibility to protect you from yourself?

I have no problem with ALS units providing this service, but we have limited resources and its getting to the point that if we add something new, we need to take something away.

Now, now, Barry, you can't use this argument. The FD has been taking responsibility for other peoples stupidity for the past 200 yrs!

  5. Well, 2 people went up and supported him..Just 4 years of employment....Being a paid EMT at Mobile Life previously and a volunteer in Marlboro w/ no experience in Dutchess other than the 4 years gets you the top job in the county!! Guess others that have 15+ years of EMS/Fire and Coordinator experience arent qualified enough....oh well!

Had to step out for errands. Missed the session, So he was approved, after all!

  6. If one were to complete the PHTLS instructor class, does this enable them to set up and teach their own classes?

Thanks!

No. It's a bit of a process. After completing an Inst. course (you must have already taken a provider coarse) you are really an Inst. candidate. You than must teach a course & be audited by PHTLS regional faculty, you then can apply to become a course coordinator. Than you can set up your own course. Where are you trying to do this?

  7. As long as next December the City Council isn't asking for more givebacks. I marched at one of their picket lines, it's a really sad situation in Newburgh. No one wanted to lose these 12 guys, not the citizens, honestly not even the council. The city is completely in shambles. The mayor claims Gov. Patterson essentially told him a variation of Ford's Drop Dead

I'm just curious if, in the spirit of brotherhood, the chief agreed to take a 5% cut over the next 2 yrs. & stand with his men. Already some council members stated they would vote against it, they're probably looking for more. Any takers on the odds that next yr we're back at the same place?

  8. So with recent talk on the internet, the IAEP is looking for transcare in the city to join the ranks of Unionized EMS. Apparently Empress EMS is the only union formed EMS in the hudson valley region, and on the IAEP Transcare website Empress is waiting on approving a new contract with a 13% incress in wages.

Heres the link to IAEP Transcare regaurding EMPRESS EMS:

IAEP dispels Empress EMS Contract Rumors

So when it's all is said and done.. Why can't EMS be unionized and as strong as a PBA, or IAFF?

Because there's to many (kids mostly) who are willing to do the job for next to nothing for either the rush or for the "experience".

  11. Amen, brother. They call it "mutual aid" when it is really more of contract for back-up service.

Somers pays WEMS more than 1/2 a million bucks a year to staff their first ambulance and dispatch them from their own facility. Since Somers is a political subdivision did they bid this out or just give the contract with WEMS?

Does WEMS a private for-profit business pay for the space they use at the district? Why are Somers taxpayers underwriting their operation?

Didn't Mohegan split their EMS operation from the FD and create a VAC that can now bill for service? Why doesn't Somers do the same thing? Then users of the system can subsidize it instead of just their local residents. How many accidents does Somers go to on 684 or other roads that involve non residents?

The whole dispatch thing makes no sense at all. 60-Control could dispatch Somers in a heartbeat. They dispatch tons of busier agencies. Its like Somers wants to be its own private little empire.

Does the WEMS crew at Somers go out of the district on mutual aid if that's the first call?

Not arguing with anything you said except WEMS is an excempt not for profit organization.

  12. Every FF I ever spoke with never passes a chance to quote from the bible, that is, NFPA Standards. NFPA 1584 took effect in Jan 2008. It became, so to speak, the law of the land within the fire service. It requires/recommends a minimum BLS level (ALS prefered if available) care & transport capability not only at fire scenes but also at all training activities. In 25 more days, Jan. 2011, it will be 3 years. My ques would be... what's the hold up.

  13. This is a subjective observation. The forum operates 24/7/365, with thousands of topics being posted and discussed yearly. A lot of these topics are to pass along information about different things, and others are posted for a multitude of other reasons, including discussion-whether intended or not. Some topics interest some, others don't. However, if you look through all the topics, there are some great posts to be found. The way a member uses to read through the new topics or postings since their last visit can sometimes also be to blame, I have noticed.

We do reconize that we have some very talented and knowledgable members on this forum, and are currently working on a program to improve and promote more informative and educational topics from some very rich minds. We hope to release more information on this soon.

    Seth

I understand & you are correct. There are topics that are certainly interesting to others (FD, PD) I primarily look for EMS issues myself, although I do read & have commented on others. There just seems to be a lot of "fluff" without much substance at times. Not to cast discredit on you or any of your staff. After 7 years and the recent fiscal issue you have proved your popularity & staying power. Providing a forum for open discussion & allowing a certain amt of venting is needed & good for everyone. Your hands off style (except when needed as in the post I emailed you about & was taken off probably before you even got to my message) means most of the site is driven by us, so any posted topics, discussion or the lack thereof falls on us. when I first joined not to long ago there was a discussion going on about the lack of incident reporting on the western side of the Hudson. As was said, if none of us report it, it's not posted. As for myself I will try to be more active in a positive way.

  14. You want to know what a real kick in the nutz is? We have almost 50 comments, and 4 pages of replies in a topic about an immature fire chief who hung truck nutz on his rig and defied his boss the mayor when told to remove them.

When we try to get a topic moving on this site about elevator rescue techniques, fire fighting tactics, scenarios, "what would you do" types of discussions, we get......YAWNS.

When we get a photo set of a new rig, some of us go on and on about the rig; where's the hurst-tool, where's the water cooler, we'll get the 50 comments and the 4 pages again.

I'm not sure why, but it seems sometimes the topics that should be getting the comments and the pages of discussion go by the wayside; and the ones that should get a few replies, and move on down the board into category obscurity, seem to go on forever.

What's up with that? I'm not saying conversation about the truck nutz should be over. What I'm saying is the next time one of our truly knowledgeable members comes up with an idea for a good discussion, maybe some of us should make a little more of an effort to participate and keep the really informative discussions going....

    Maybe that will start happening?

In the meantime, back to the nutz.

I gotta agree with you on this one Cap. After reading the first few posts on this issue I was done. 50 replies & over 4 pages! If it was an actual conversation between people many, I'm sure, wouldn't have wasted their breath on the stupid SOB other than he's not fit to man the knob much less command. It sounded like old women gossiping over the fence after hanging out the wash. I look over the posts daily & there are not many real interesting or professional topics. Hope this changes.

  15. Right! The FDNY would of had ladder companies searching and maybe a few tower ladders hitting the building from the outside closer to the ground. Plus, over here, we would have started an interior attack(why didn't they use the building's standpipe system?), as well as search and rescue.

And, I totally agree with you there. People in this country do take building fire safety codes for granted. Plus, our fire service, as far as (mainly)response times(half of those 100 pieces of fire apparatus should have been on scene long before the video was taken and most of the building was engulfed), amount of fire apparatus on scene, fire attack methods, firefighter safety, search and rescue methods, and even high-rise firefighting in general, is much more evolved and advanced firefighting, as compared to many fire departments in parts of Europe and Asia. Not to say that these departments are still using horse-drawn steam engines, but firefighting in the U.S. is really one of the best firefighting industries in the world.

I guess it also helps in situations like this that our country is a Democracy.

Maybe they didn't even have a standpipe system. They obviously didn't have a sprinkler system.

  16. Can a conscious patient refuse medevac? Would you just document it on the PCR?

They can, just as they can refuse going to the hosp, C-Spine, IV etc as long as they are able to understand all of the possible consequences IE an informed decision. If you look at the back of the PCR,refusal is for Treatment or transport. It's not that simple however. Most civilians don't understand the process. Most think it's just part of the service, (until they get the bill) just like calling FD to put out the barbecue fire that gets out of hand. Now take out west in most of the wilderness parks. They require you to sign a waiver agreeing to pay for any & ALL costs associated with any rescue &/or recovery operation which might be needed. And those costs are not the small change we're talking about here.
