  1. No need for them. All they will be doing is the transport. Thats why they have this team(White Plains) so they can drop a man down to the PT and start ALS care before you bring them up.

Does that mean the "Rescue Medics" will be continuing patient care onboard the Transcare rig on the way to Westchester Medical Center? The reason I thought the local EMS should be integrated into the drill would be to work out the transfer of care to the transporting medic.

  2. The closing hasn't taken place yet. The recent log jam in Albany is most likely the cause. No one doubts it's going to happen. The former Fishkill crews are turning out of the station on Middlebush now. Currently Medic 18 and Medic 21 are there. I haven't seen anyone in a Transcare uniform driving an Alamo rig yet.

  3. The vast majority of patients in Yonkers are transported to Yonkers hospitals.

    This is because:

    1. NYS protocol.

    2. Most patients express a preference for one of the local hospitals.

    3. Transporting to a local hospital allows a shorter turnaround time for the ambulance.

    4. The more patients a service brings into a hospital, the more likely it will be called for the outgoing transfers.

The patients that are transported to Jacoby or WMC need the specialty services that the local hospitals can't provide:

    Trauma, Burns, High risk pregnancy.

The "cure" for trauma is surgery. For surgery to be effective, it should take place during the first "Golden Hour" after the injury.

Despite what you see on "ER", surgery is not done in the Emergency Department. Trauma centers are prepared to fire up an

operating room within minutes of a trauma patient arriving at the hospital. A local non trauma hospital might need hours to assemble an OR staff. They may not be able to do neurosurgery at all. Therefore, bringing a trauma patient to a non trauma hospital delays definitive care and leads to poorer outcomes.

  4. I worked for both WEMS and Transcare in my day. While you will do alot of transports with WEMS, you will gain alot of knowledge from the transfers if you apply yourself. I know they were mixing both GLS and ALS rigs for transports back then. I will also tell you WEMS has an excellent benefit package should you go there.

Transcare is also good. Yes you will have to work transport until you get some experience under your belt, but once you get into the 911 system, you will be doing all 911 jobs.

that being said, one thing about both of them is that the road people as well as the supervisors are really good people and good to work with.

I would also suggest you try and get into a VAC to get some experience under your belt.

    Good luck,

    joe

GLS = Geriatric Livery Service :) j/k Joe!

  5. With very few exceptions, having a scanner or other two way radio in your car is against the law. That being said, I have never heard of anyone being charged with this offense if they weren't doing something illegal or stupid at the time. Use common sense. Don't flaunt it in front of the police. Don't use what you hear for personal gain or to compromise someone's privacy. Don't buff calls that you have no business being at. I've had a scanner and a couple of portable radios in my personal vehicles for many years and no one has ever said a word to me about it.

  6. i think the guys did a fine job i also think that with a call like this the ems side really need's to step it up and not take there time getting to there bus i watched them at the same time watch all the cops fly down rt.9 and 1 of the rfd chief's i think as the first crew of the amb. took there time to finsh buying stuff and finsh what they were doing befor they responded not calling out the time from dispatch to the time the bus was started and moveing how nice to call responding from a hand radio and not the one in the bus just because your right up the street dosent mean you can take your time and they werent at there fishkill st.

I'll reply to this when I get the english translation.
