    It sounds like Croton EMS got there quick enough, thankfully. I have been impressed by their response times compared to this time last year, where a patient would be waiting 10+ minutes for a mutual aid ambulance to arrive. Not to start another debate about that subject or anything.
    Great work done by MTA PD workers, it's great to hear about these stories. Prayers are with the family of the newly born for a healthy future, and lets hope this doesn't happen again.

    NYS Health Law Article 30 requires all ambulance services to recieve a CON (certificate of need) from the Department of Heath. To get that you have to prove that there is a need for an ambulance service in an area and that an additional ambulance service would not (financially or membership drain) hurt the existing services. To prove need, you would have to show that calls are either not being covered or are being delayed. Since the area is fully covered by ALS services with average 4 minute response times you will not be able to prove need. An additional service will financially hurt the 2 existing services in (MV, NR, & Pelham). You could also hurt the membership rolls of Eastchester, Scarsdale and Larchmont/Mamaroneck VAC since many of there members come from NR.
    Since each of the communities already have established services and are dispatched by the local police (most via a service contract) you will no be able to recieve calls.
    Finally, Most VAC's are having major problems with staffing, instead of watering down the pool, why don't you join one of the existing VAC that could use the help.

    I completely agree with you BNechis about apparatus and staffing levels... But that's New Rochelle and while it may not be an issue there, what about other jurisdictions that struggle to send a fully staffed engine out on calls? I guess whether to send FD or EMS really depends on the dynamics of the individual system to decide what works best.

    We have 2.5 staffed ambulances and 8 staffed fire apparatus. It is often less of a problem for FD to be there and in most cases the call lasts less than 10 minutes.
    As Antiquefire points out, you cant bill for these (or risk losing your medicare payments). So thats not an issue.
    Since all our firefighters are EMT's. We expect them to medically clear the person 1st, then pick them up. It is very rare that we need transport equipment and we have it or if really needed we can call for an ambulance. normally we only need it if they are injured and need to be transported to the hospital.
    A very good point and we do take advantage of that.
    Another advantage is many of our lifts require forcible entry, which EMS would need us for. Even better is in many multiple dwellings we have Knox Boxes....So the FD has the keys to get in. This is particularly true in all our highrise Sr. Citizen buildings.
    Finally, we have a patient assist system that our crews can document the situation and is forwarded to the Office for the aging (and others). This has reduced repeat calls, gotten meals on wheels and home health care services, in some cases special medical equipment and sometimes placement in long term facilities.
    I had one call for a person who was on the floor, he had a helth care aid, but was to big for them to lift him. We found no injuries, lifted him back into bed and noticed many cigarette burns in the bedding. I reported it, thru the system and they were able to get the health care service to keep the cigarettes away from the bedroom.
    The fall was incidental, but the other was going to create a big issue.

    And yet everyone (including myself) applauded members of the FDNY for backing up and assisting NYPD officers in Staten Island who were faced with a large unruly crowd.
    Who's guarding the hen house? The other Troopers working. Those Troopers most likely would have been assigned to the fire scene anyways.
    If a Firefighter taking one of my guns to help me in a firefight saves my ass or someone else's, I'm good with it. These Troopers didn't take the hose from anyone's hands, they were asked by the Fire Chief to help. Any cop would have stepped up and did the right thing.
    The simple fact is web the crap hits the fan, emergency service providers step up and do the right thing for the communities we serve and for our fellow emergency service providers. These Troopers were not tasked with an interior attack, they were tasked with spraying some water on a fire from well outside the structure because the FD didn't have the manpower. If I was a homeowner, I'd be pretty damn grateful.

    A local story that's hitting close to home for me: File under "The System Worked!"
    On Monday, April 30, Ron Immel, EMT/Supervisor for Good Fellowship Ambulance Club, responded to a medical call, along with Chester County Hospital Medic 91.
    While packaging the patient, Ron went to secure Medic 91's car when someone heard him collapse. Immediately, personnel found him unconscious behind 55's ambulance. He was found to be in V-Fib arrest,...CPR was initiated and he was defibrillated at the scene back into a sinus rhythm, ultimately regaining full consciousness upon arrival at CCH!
    As of today, after a battery of tests, the cause was unknown and he is scheduled to be discharged tomorrow.
    I can't attest to Ron's overall state of health, but as emergency providers, we work long hours, endure rapid escalations of stress, and probably don't eat the healthiest of meals when on-duty.
    Personally, this time last year, I was 270 lbs, had tremendous back pain (in part due to advancing arthritis) and lost so much sleep I was nodding off at work. I'm now down to 225 and I have a few more to go (so says my doc ), but its not all about me - we all need to look after ourselves. We all won't have Ron's good fortune to be surrounded by a lot of competent care providers.

    I can understand all of the thoughts on how to prevent another tragedy like this from happening, but the fact of the matter is, a great man is no longer with us, along with his wife and two teenage girls who barely began thier adult lives. Not to mention his son who survived the fire, but will have to live with out his home and more importantly his family. Pray for the Sullivan family and end the discussion. This happened one day ago!! Discussing the details and giving suggestions and ideas so soon is very disrespectful to the Sullivan family.

    The BFD brothers and sisters showed great heart and strength in not only attending thier own fallen brothers wake but also Lt.Tyler Rush of Elmsford. That spoke tremendously of the Briarcliff FD and their resolve and support.

    Another reason why a complete county-wide consolidation plan for fire services makes sense. In the long run, it will save communities tax payer money and allow for efficiencies and better response, with properly trained personnel at fire/emergency sites.
    I know that Barry is an expert in this and I am sure that the time spent on his own dime was greatly appreciated by everyone in the City of Rye. However, it appears as if this solution may only be a "band-aid on an open wound" (although the "Open Wound" may take YEARS to ever come to being)

    I don't have my contract IFO me, so I will do this from my memory, which has a high failure rate. IF you do at least 24 hours a week in dispatch, you get an about $2.50/hour above what an EMT with the same credentials/Hx would make. Your other hours would be worked on the street. IF you work LESS then 24 hrs a week inside, you only get the extra money for hours actually spent dispatching. Empress sends you to the EMS class.
    There is an established system for counting previous years as a paid staff and/or volly, something like 2 or 3 years in another place earns you a one year grade increase at Emnpress. Again, it is in the contract so everyone gets the same deal.
    So let's say a new EMT starts at $10.50. Then about $12 after probation, then the $2.50, plus maybe a $1/hr or so for previous experience if you can document/prove it= about $15/hour, plus the benefits, and about a 3% raise, give or take based on what contract is in effect.
    I gotta say, it is a VERY demanding job, and most of our dispatchers are VERY GOOD at it.

    The things you are saying that are already passed into law...they were done so on the backs of unions pushing for them. And while they apply to you...let me know how you make out when its happening to you and you try to get it rectified.
    I give you a golf clap on your speech about unions being crooks and you can post youtube videos. I wonder how they come to be...I only wish (if I actually had the time or care to do so) find some of the things I've dealt with in my career that without unions would have just gone unchecked. But hey...what do I know.
    Do yourself a favor...I highly suggest you do go non union...or at least wait 2 months before your next urinalysis...cause your either smoking something or someone has been slipping something into your fox news kook aid.

    6 years ago I wrote that what it would take is either a major disaster or the state forcing the issue.
    Well the state has given us a tax cap, which is forcing almost everyone at a minimum to consider how it spends its money. I know of a dozen depts that are already cutting back. A few considering cutting manning. In the past year 2 depts had lay offs, another had threats of it. We have combo depts. that have zero volunteers, but think they can cover the town with 2 or 3 career members.
    We have publicly seen depts. lose homes because they can not put a team on the field, others because they can not find the hydrant, others who have lose members because they did not want to train and follow OSHA laws. We have a local fire district that want to build new stations, after the public has said no, and the dept has no plan to staff the house. A few weeks ago I was at a town council meeting and the FD "leadership" stated: "we know we suck, but as long as the public does not know, it fine"....
    Last week at a county fire marshals training class 4 different fire inspectors from 4 different towns asked: "what should the town do when the FD can not get a rig out the door or even 1 member to respond to a fire alarm"?
    This is not an attack on firefighters, officers, volunteers, career, commissioners. It is an attack on a system that is a total failure. Yes there are some great depts here...but if you average them out county wide the numbers suck.
    We have hit the iceburg, we all know there are not enought lifeboats but instead of working to save what we can, we would rather go down with the ship.

    I worked at Empress before, during, and after the organization of the IAEP...so I have seen both sides...the increase in pay was nice...but I was a young guy then, I thought the pay was decent to start with...especially compared to the agencies that covered my home...(I lived in Dutchess, commuted to Westchester for 7 years)..but perhaps the biggest "benefit" was knowing that even though you were working for a FAMILY owned private BUSINESS, they could not get a wild hair up their rearends and just fire you on the spot...it HAD to be justified. And of course - as mentioned, it does offer protection to those who operate with the anchor down...problematic employees who truly didnt deserve the job. There was also the PRIDE associated with belonging to a "Union Family"...I mean, we were working in YONKERS for crying out loud - if THAT aint a Union Town, I dont know what is!
    I still have my little IAEP pin on the band on my fire helmet...just 'cause.
    Now would Mobile Life truly benefit from organizing? I have no idea...I dont know enough about them. I will say this; MLSS employees have the highest morale of ANY private I have ever seen. I deal with them now in Poughkeepsie and I have absolutely nothing negative to say about them at all..they are a very professional, well run company and the most important thing in my book - THEY HANDLE THEIR JOBS AND DO IT WELL. I hesitate to say we are spolied by them here...but damn close. LOL.
    BUT if EMS wants to be seen as an equal service with police and fire, they need to UP the ante...they need to start holding themselves to a higher standard, be more professional, and market the JOB as a true equal 3rd service.

    The employees at Care1 are fantastic. I think the topic of discussion is the management, not the field staff. Ive had the opportunity to work with the EMTs and Paramedics for years and they are nothing less than dedicated,respectful,and hard working. The large majority of us have worn different patches, different color uniforms over our careers, the company does not make the employee who they are. I agree the reputation of this agency far proceeds the staff,and some people can not see past that. As I see it, the problems are largely internal and at a management level. No company is without it's own set of issues,I just think that not only are a large number of new hires are leaving, but an equally large number of field staff that has been there for years, through good and bad and all the changes over the years it needs to be evaluated.
    I really believe the field staff has kept this company going as long as it has. It's the faces of the crews that the facilities,patients,and family see and it is the field staff they interact with and it's because of them they call back. It's unfortunate that management ignores this and chooses to step on them and treat them as they do. Remember ladies and gentlemen, we do the same job, we are on the same side of this. We can't lose sight of this.

    Pretty darn good drill with familiar faces and some terms and comments I think I've heard before. He must have had some pretty good instructors along his path.
    Couple of things also to remember...and again...he did a great job. Always be ready to deal with or have in place a way to secure the door. There will be times when you shock load it or gap it..that it may open at that point. If you're in a common hallway...do not open the door without a charged handline in place.
    Also peel back your glove..you're not going to feel heat through it and you'd be shocked when I teach if I ask..if the door is hot do we open it? and students say no. The answer is yes..but you have to be prepared for what can occur...be it have a handline...just open it to take a peak if your searching and control it. We're firefighters...its to open a hot door under the right circumstances.
