    e. Remain with patient at all times
    f. Avoid challenging personal space

    (3) Implemented by law enforcement authorities, if at all possible
    i. Types of restraints
    (1) Wrist/ waist/ ankle leather or velcro straps
    (2) Full jacket restraint

    Lesson Outline:
    Physical restraint of the violent patient
    a. Improvised restraint devices (materials from the ambulance)
    b. Commercially made restraints (leather or nylon, padded for comfort) that prevent
    movement of the arms and legs
    c. Make sure you have sufficient personnel (police assistance is required); minimum of
    five people.
    d. Move quickly to restrain the patient.
    e. Leader should maintain verbal contact with the patient (even if the patient is not
    paying attention).
    f. Check the patient’s peripheral circulation to make sure the restraints aren’t too tight.
    g. Document everything in the patient’s chart.
    4. Skill Drill: Restraining a Patient
    a. Assemble four or five rescuers and have the stretcher or carrying device and soft
    restraints (wide cloth or commercial leather restraints) nearby (Step 1).
    b. Designate a leader who will communicate with both the team and the patient.
    c. Assign positions to each member: four extremities and the head (Step 2).
    d. If possible, corner the patient in a safe area with the least obstruction and no glass
    (Step 3).
    e. On the direction of the team leader, who will be talking to the patient calmly, move
    together toward the patient (Step 4).
    f. Each team member should grasp the assigned body part and carefully, with the least
    amount of force needed, bring the patient to the ground (Step 5).
    g. Carefully place the patient on the stretcher or carrying device in a face-up position
    (Step 6).
    h. Tie the patient with soft restraints at each wrist and ankle as well as over the chest
    and pelvis with sheets (Step 7). If the patient is spitting, place an oxygen mask or
    surgical mask on his or her face.

    Restraint
    When necessary for the patient and/or crew’s safety, in addition to the straps on the stretcher, a patient may be restrained using the least amount of force and restraint necessary.
    Remember that the most effective restraint device is a calm, firm, professional demeanor.
    The first step is to request the police to place the patient in temporary police custody. In the absence of the police, the EMT should attempt to gain permission from the patient’s parent (if a minor) or guardian, or contact medical direction for advice. In the event that neither the police nor a parent or guardian (for a minor) is present, the EMT may restrain the patient if it is safe (for both the crew and patient) and you determine that the patient may be a danger to himself or others. It is preferred that there be at least three crew members present. If the crew is in danger, the appropriate thing to do is retreat!
    Patients should be transported with a crew member of the same gender, if possible. Patients not in police custody should not be handcuffed. Patients should never be transported face down or with their respiratory capacity restricted in any way. All restrained patients must be continually monitored.

    First..one of the most important things to keep in mind is you must use the minimal level necessary for the safety of your patient and yourself. Meaning from verbal and if need be restraint. I'm not sure what some of you are using as knots and around the wrist for a restraint if needed with a kravat...but I've never had one slip. Guess the rope rescue technician does come in handy in other ways.
    Few other things to keep in mind:
    No one, including in police custody should be left in the prone position.
    A very good technique which I use if its a case where I need or may need IV access is the one up...one down method. Meaning one arm is secured in the up position, the other down.
    You have to look at your stretcher to find a good hard point to tie off to as they are not made for this. Any movement will able them to get to some part of the body to untie or get out..ie mouth other hand.
    A clove hitch around the wrists with the kravat left as wide as possible...(I use and highly recommend wide restraints to lower the risk of potential nerve injury) has always worked for me. Also do not allow them if you can to flex their wrists when tying or it will be hard to keep it snug. That's the good thing about the clove hitch (or any hitch for that matter) is that it tightens under load.
    Bottom line is documentation. And I mean detailed documentation. From beginning to transfer. How the patient presented in detailed. How things esculated, what you did to right up to the point that you opted for restraints and I always document "decision was made to use a soft restraint for the safety of the patient and crew." I document what I used, what I tied, where and that I continually checked to ensure that extremities had circulation.

    What happens when this gun gets to a persons home? Are they going to secure it properly? What happens if they have kids? If they secure it properly, will they be able to get it out by the time it is needed?
    If they leave it in their car to run in to a Quickie Mart that sells alchohol, what if the car is broken into?
    In large crowds, how are they going to secure their weapon?
    Are these people trained to use a firearm in an active shooter situation? How are they going to react to stress? Can they ensure they won't hit innocent civilians? If multiple people pull out guns in a large scale enviroment, how will they identify who is the real shooter? Isn't that what we have police for?
    When a police officer fires his weapon, it often costs millions. How will these civilians protect themselves both legally and in a civil case?

    And are we still worried about the British invading? And when the Constitution was written, did the authors ever imagine automatic weapons? Or were they just thinking about muskets?

    Legislators should worry more about funding more police officers and more investigators to deal with these types of situations, get guns off the street, and tracking the guns that are out there and who's hands they are in.

    I'd be surprised if a law such as that proposed would allow a privately owned public access property such as a stadium, deny entry to persons legally carrying. This can be viewed as either allowing persons with guns to enter or not allowing owners to deny entry to a person legally exercising their right to carry. Not that this isn't fraught with troubles, given that there are many people who have yet to break the law that are carrying. But the whole issue can be a matter of prospective: are we allowing them entry or protecting their right to entry?

    The Indiana Senate has proposed a bill that is very close to becoming a reality. The bill is "Senate Bill 292" which would allow people who are registered to carry firearms to bring them into public places.

    I find this to be a scary bill. It allows firearms to be present in location that are often home to fights, drinking, drug deals, etc. I would not feel safe sitting at an Indiannapolis Colts game knowing that people are drinking (some heavily) and some people have tailgaited for hours prior to the game and now have guns. We have all been to sports game where fights break out and punches are thrown because one drunk fan says to another fan, ' Hey as*****! The (sports team) sucks!", or something like that. Now instead of a pu8nch or two being thrown prior to the arriva lof security or a good samaritan breaking the fight up, there will possibly be gunfire! Thats ridiculous.
    The bills sponser, Senator Tomes feels that the guns will make public places safer because there will be more people with guns who will act as "first responders" in the case of an emergency...yeah ok pal, nice try!
    For some reason, I no longer want to go to the Super Bowl next year...someone else can have my ticket. Make sure you bring your gun just in case you spill some beer or popcorn on someone!

    It's a confusing article. It starts out with this "political bantering" comment. Where did that take place, in the firehouse amongst the members? Did they get into a heated political argument, because towards the end of the story the firefighter in question says he was so distraught that he felt he couldn't perform his duties, so he went home sick. He also said he voted for Giffords.
    He also retired apparently. So he's gone.
    Not much of a story here really, much ado about nothing in my opinion.

    Sounds like they were responding with a 'scene support truck' and look what happened to the crew. Maybe this is why WCDES scene trailers are such a mystery and no manpower to man it. The county is protecting the mental health of it's firefighters.

    Accuweather.com states that there may be a good chance that the northern lights will be visible in upstate NY, and New England.
    They are also predicting that this could possibly interfere with some radio communications.

    The NYC Urban Area Work Group (which includes NYC, Nassau, Suffolk, Westchester, Yonkers, PA of NY & NJ) about two years ago had all members agree to purchase a medical evacuation vehicle - (similar to a plan in Northern NJ) - the idea was to buy identical vehicles which would be operated locally but be available regionally - each partner agreeing to buy at least one & NYC buying a few. The spec has not gone out to bid yet but should soon. I don't see the vehicles being delivered for some time yet.

    Billfitz makes a maajor statement he He doesn't know re. Why would an IC call for these resources if
    1 He doesn't know if they exist or is it rumor
    2 He doesn't know what types they are or what they carry
    3 He doesn't know How to ask for one
    4 He doesn't know response time
    5 He doesn't know manning or skill level of responders
    If he did call for one of these resources, he is very likely, when it gets to the scene, to find out that it is not what he needed and the situation status has worsened in the meantime.
    If you were an IC, would you call for something this questionable?
    I would go for a sure thing as opposed to a guess.

    ' I have been down there alot lately and have seen trucks and stuff that looks brand new too... there is a scene support truck too? looks like a 10 wheeled modified box truck with a 4 person cab... i can't say for sure i didn't take a really good luck at it... but it would be nice to know what is there if we need it... why can't the tech rescue guys or hazmat guys staff it as a volunteer thing? just get it there type of thing ?     As A Member Of The Public Safety & Security Committee I'd Like You To Be Aware That An Enormous Amount Of Federal Funds And Taxpayer Dollars ( Millions + ) Were Spent On Vehicles For Westchester County DES At Grasslands Reservation And MOST Of These Are Housed In A Special Garage Collecting Dust Because They Can NOT Be Manned Or Were NOT Needed. Numerous Attempts By Fire Service Personnel To Inquire How These Vehicles Can Be Special Called And/Or Utilized Were Met With SILENCE. This Is A Misuse Of Taxpayer $$$$ And Federal Funds And Someone Should Be Held Accountable For This Horse & Pony Show Before The Media Is Involved In An In-Depth Investigation. i.e. 1) A Field Communications Unit And Paid Lieutenant That ONLY Appear At Parades And Public Safety Expos Rather Than Responding To Major Fires, Etc. 2) Scene Support Trailers That Have Been Parked For 6+ Months And NO ONE Can Explain Their Purpose. 3) Collapse Rescue & Support Vehicles That Haven't Logged 500 Miles Since Being Purchased From A Long Island Fire Department.

    I highly disagree with you posting the department's name for starters. Second what agency are you in let's see how good you guys are. Everyone has issues there is atleast one dept in that same area that's worse than the one you listed, but everyone has issues no one can be as perfect as your agency.
