  1. NYS allows EMTs to use it, but your agency must be approved by the state and the region to use narcan

    I submitted and recieved the approval through the Hudson Valley EMS Council for my department to use intransal Naloxone.

    Used it once so far and it worked like a charm....... :rolleyes:

    20 yof unconcious CPR inprogress (not sure why CPR, she was moving air fine). 1mg each nostril within 3 minutes she walked to the ambulance.

    We all know the epidemic with the heroin spreading like wildfire. Everyone says "just let them die", I explain 1 thats not our job and, 2 it is much easier to push a bristojet for 10 seconds then to do CPR for 30 minuntes....

    I think you will see more skills come to the BLS side of EMS within the next couple of years.

  4. While I can not find it, Phoneix started a pilot on the Intranasal Narcan. The numbers were actually astounding. The revival or success rate was near 97%. Getting there in a timely manner hopefully your only findings are respiratory arrest, thats where the good old BVM and Narcan would be huge.

    Obviously we know when bad batches come into our jurisdictions, the OD calls rise.

    I think this would be a asset on a BLS level.

  5. You have to operate professionally to be considered professional. I hardly consider all the sloppy EMT's and paramedics I see professional and it doesn't matter whether they collect a paycheck or not.

    I think you may have missed my point.... Whether you are PAID or Volunteer you should be considered professional, not just because your are paid. I understand and have seen the sloppiness first hand, but my point was, collect a pay check or not you should be considered professional, whether you conduct yourself and your skills in that matter is totally different.


  6. About the whole ALS/BLS thing

    If you have a priority call and a BLS truck is 5 minutes away, and an ALS truck is 30 minutes away, isn't it better for the patient to send the closer unit, then intercept en route, then have a patient wait 25 extra minutes with no EMS at all attending to them.

    Now if you are saying that they should be intercepting with their own company, yes that is true.

    Actually that is not what I am talking about. To send a unit across town and or county when there are other muncipalities or agencies that are contracted or not even contracted but have the proper level of care for the call is absurd. This is negligent, not to mention extremely bad for a agency to send a unit blazing saddles so they can what? Please a contract or facility.

    This extends back to when they had their Narcotic issue and they could not operate on a ALS level. How many other agencies had to do ALS intercepts with them? Plenty...........

    If it was simply sending a BLS unit to start patient care I would 100% agree with you.

    First off, what is "stop the madness" all about? Long story short, i used to work for Transcare, yes I had issues with the company due to their lack of structure and consistency of training.

    Their method of training was instead of being with a Field Training Officer, was thrown with different crews each day. How is one to learn the business by someone other than someone

    whom is dedicated to training and evaluating new-hires. Secondly, I did not bash or bad mouth the company. Yes indeed, many of their ambulances are in disrepair as was some of their

    equipment.

    Thirdly, Obviously Transcare covers most parts of the Hudson Valley and New York City. EmStar is from Pennsylvania, and is just starting to move further east, obviously if they have

    taken Care1 EMS under their wings. I don't know much else about the company. Other than what I looked at on their website. It would be fair to say however, they cover just as much

    territory as Transcare does.

    On a closing note, I found this reply of yours very interrogating. Why reply the way you did in open forum? If you had an issue, we could had sorted things out back-channel.?

    I will not get into a pissing contest with you on such a forum. Just please research the facts before you put up "factpinions", everyone has issues with vehicles, and TransCare is in fact much larger than you know.

  8. EXDC, I don't think there is any problem with EmStar. I checked out their website, researched the company some. By the looks of it seems this company may soon become a larger

    Commercial Ambulance Company than that of Transcare. Advantages of EmStar, obviously newer buses, while Transcare desperately needs to upgrade their clunkers.

    Stop the madness......... We forget TransCare is already all ove the East Coast. As long as they send the proper resources to calls the I could care less.

    Sending a BLS units to a semi or unresponsive patient is NOT acceptable! So far I have seen no change in service with the exception of the same trucks with a different wrap.

  9. Wonder how much they have spent the last 5 years on re-lettering all their equipment. Maybe they should get velcro, like some EMTs/Paramedics do on their uniforms.

  10. TransCare does in fact run a "Organ Donor Team". TransCare utilizes a Ford, Expedtion/Flex/Explorer and a van for this program.

    The team runs all over the Metro area and has been seen as far North as Vassar Brothers in Poughkeepsie. The team runs in conjunction with a lot of the hospitals in the city and Westchester area.

    As far as running lights and sirens anyone in the Emergency Services knows what it takes and how time sensitive harvesting organs can be. It is rather ignorant for anyone to say that ANY company is abusing the lights and sirens until you know for a fact what the units job description is.

    I have attached a picture of one of the vehicles, they all have the same exact sceme.

  14. On my way home for work I heard Arlington get toned out for a reported fire at Cherry Hill Apartments. I had to buff some photos......

    I must say for the amonut of staffing, Great Stop by AFD..... Anyone who questions staffing should have been there. There was plenty of hose to be pulled and fed as well as ladders to be put up.

    Awesome stop AFD, this could have had a very poor ending if it wasnt for the quick and agressive attack.

    The chaueffur on Tower 2 put that Tower in some tight place.
